We look forward to welcoming you this summer. Below you will find a list of forms for students accepted to our 2018 Salisbury Summer School. Please review the documents and fill-out and return any applicable forms. If you have not yet applied to the program, click here.
Both the elective course enrollment and the questionnaire are very important because we will be unable to finalize your child's academic schedule without them. We would also like to draw your attention to the memo regarding students leaving campus on weekends during summer school.
Accepted Student Forms:
REQUIRED HEALTH FORMS (must be submitted by June 15)
The Salisbury Summer School Health Center uses Magnus Health, a secure and easy-to-use Web site for submission and storage of your child’s medical information.
UPON ENROLLMENT: Please e-mail firstname.lastname@example.org with the following information, allowing us to establish a Magnus Health account for you/your child:
- Student name
- Student date of birth
- Parent/Guardian name
- Parent/Guardian email
- Parent/Guardian phone number
Once the Health Center receives this information, parents will be contacted with important Magnus Health account information, as well as instructions regarding the use of the electronic medical record system.
You must access your child’s Magnus Health account in order to obtain and submit the required health forms by June 15. Incomplete health forms may result in restriction from summer school classes and activities.
Immunizations must comply with State of Connecticut regulations and students must provide documentation of a physical exam within 1 year of registration.
Salisbury Pharmacy handles all of Salisbury Summer School’s medication needs. All parents are asked to complete the Salisbury Pharmacy Form linked here, in the event your student requires medication while at School.
If your student will be taking daily medication while at summer school, please do not fill the prescription at home within one month of your child’s arrival. Rather, please send all prescriptions directly to Salisbury Pharmacy. Please refer to the “Medication Order Form,” which can be printed from your son’s Magnus Health account, for more information regarding prescriptions.

Due to the nature of our summer program, the Immigration and Naturalization Service (INS) has confirmed that an I-20 form and an F1 student visa are not required to enter the U.S. for attendance at the Summer School. Visitors to the Salisbury Summer School may enter the U.S. on a Visa Waiver or B-2 status depending on the country of origin . A letter, detailing the purpose for the visit to the U.S., will be mailed to the student after enrollment in the program.
